Julie, for a second marriage you would put your full name - first, middle, maiden, married. However, there's no law that says you must use your married name. If you want to leave it out, it's your choice. Using the full name is proper, but using a partial name is not improper. Consider whether people will know who you are though if you leave it off. Will all of your friends recognize you by your maiden name alone? If so, then leave it off if you like.
For your gown you may wear any color that you like, including white. Etiquettely speaking second time brides have always worn ivory or beige (something off-white). However, it's becoming much more acceptable for second time brides to wear white in today's weddings. If white is what you want, and etiquette is what you are concerned about, you can rest assured that white will be fine.
If you ever look back into the history of white wedding dresses, it was created by Queen Elizabeth for her own personal satisfaction. Before that, women wore their Sunday best as a wedding dress, so wear whatever color you choose!

You can wear whatever color you want. In the 60's 70's they said you could only wear WHITE if you were a virgin.....well these days, most if we were going to be honest would be wearing anything but white! LOL. So if you want to wear white you go ahead. I wore Ivory because it looked so much better with my skin tone. I would consider that more than anything else what looks the best! Good luck!
